Medical Form

Once you 'submit' this is the e-mail you will receive. The form is a set up a little weird...the reason my medical form still said 'no' for the submission status is that I missed a required field towards the botom and didn't scroll back down after I hit submit. I just thought it went thru. Anyways - here is the message when the form is successfully submitted:

Thank you for completing the Medical Form.
We have received your form and our medical team will begin the process of reviewing it. We will notify you if we have any questions or need any further information. You can check your “My Avon Walk Center” to see status of your form (this will update after the medical team has had a chance to review the form).
